Hospital case management and discharge planning.
*Profiles must detail/be clear in regards to discharge planning
Internal floating within the department

Required:
NYS RN license
NYS PRI Certification
1-2 Years Acute Care Discharge Planning Experience

Preferred
Hospital Transition of Care Discharge Planning
1-2 Years in Acute Care Discharge Planning
