Job Details

We are seeking a dedicated Physical Therapist (PT) for a Skilled Nursing Facility (SNF) / Long-Term Care (LTC) environment. This role focuses on providing high-quality rehabilitation services to residents with varying levels of physical impairment, chronic conditions, and post-acute needs.
Setting: SNF/LTC with emphasis on patient-centered care and functional independence
Responsibilities include evaluating patients, developing individualized treatment plans, and delivering therapy to improve mobility, strength, and overall quality of life.

Discipline: PT

Shift: 3x12 hr days

Start: ASAP (may be flexible)

Hours: 36/week
