Job Details

We are seeking a skilled Home Health Physical Therapist to evaluate, treat, and manage patients in the home setting. The PT is responsible for developing individualized plans of care, providing direct treatment based on physician orders, and educating patients and families to support optimal recovery.
This role requires strong clinical judgment, knowledge of evidence‑based practice, and the ability to modify treatment plans according to patient needs and Medicare/Home Health regulations. Responsibilities include OASIS assessments, point‑of‑care documentation, interdisciplinary coordination, productivity management, and participation in Quality Assurance and Performance Improvement initiatives.
The PT will oversee and supervise Physical Therapist Assistants and may supervise CNAs when appropriate. Additional duties include scheduling efficiency, accurate mileage logs, participation in care conferences, and adherence to agency policies and state licensure standards.
