-Assessment: Performs assessments & reassessments per policy & procedure or as warranted by patient condition; demonstrates ability to make sound independent decisions. -Development & Implementation: Initiates, prioritizes and implements the plan of care and reviews and revises the plan as indicated. Collaborates with patient/family & interdisciplinary services; utilizes resources appropriately to support the needs of the organization/patient/family. -Evaluation: Monitors and documents response to clinical interventions. -Education: Facilitates/coordinates/provides/evaluates education specific to patient/family needs and as appropriate to the care, treatment and services provided during their stay and at discharge. -Leadership: Delegates tasks to licensed and non-licensed personnel; functions as a mentor/preceptor; demonstrates/encourages/facilitates teamwork; identifies and appropriately communicates opportunities for improvement; participates in quality, safety and service process improvement efforts and implements practice changes; complies with regulatory standards; participates in the Practice Governance model.
