New grads would be ok but would prefer prior SNF clinical experience. OTR would have PRN support. Therapist would complete all evaluations, recertifications, PN and discharge summaries and manage the POCs with assistants (OT). Comfort with utilization of tests and measures.
Responsibilities:

Evaluate patients’ functional abilities and develop individualized treatment plans

Provide hands-on therapy in accordance with physician orders and facility protocols

Document patient progress and maintain accurate treatment records

Collaborate with physical therapists, speech-language pathologists, nurses, and other healthcare professionals

Educate patients, families, and caregivers on techniques to support continued care and safety

Ensure compliance with all state and federal regulations

Qualifications:

Current Occupational Therapist license in the state

Graduate of an accredited Occupational Therapy program

Strong interpersonal and communication skills

Ability to work independently and as part of a team

BLS certification
