Job Details

Start: ASAP
Role: Care Manager / Acute Care Manager
Primary function: Care coordination for acute-care patients, including coordinating services, discharge planning, collaborating with physicians/nurses/social workers, and helping ensure patients receive appropriate care across the continuum.
Work schedule: 5-day workweek. The department operates 7 days a week, so your days off will vary and may not always be consecutive.
Patient ratio: Approximately 1 Care Manager : 25 patients
Floating: Must be willing to float to the Laguna facility.
Dress code: Turquoise scrubs or business casual
RTO: Maximum 7 days of requested time off.
