• Both Days/Nights are available.
• The LPN delivers safe, high-quality, and compassionate patient care in accordance with facility policies, procedures, and professional standards. 
• The LPN provides direct and indirect care, coordinates with the interdisciplinary team, and ensures optimal outcomes for patients.
  
• Perform comprehensive and prioritized patient assessments, notifying providers of abnormal findings.
  
• Develop and implement individualized care plans in collaboration with the healthcare team.
  
• Administer medications and treatments accurately and timely, following all safety and documentation protocols.
  
• Perform nursing procedures such as IV starts, EKGs, blood draws, and wound care in accordance with facility policy.
  
• Provide ongoing patient and family education to promote understanding and participation in care.
  
• Accurately document assessments, interventions, and patient responses in the EMR.
  
• Respond promptly to emergencies and initiate appropriate resuscitation or intervention measures.
  
• Maintain infection control and safety standards throughout all patient care activities.
  
• Assist with patient transfers, procedures, and diagnostic tests as required.
  
• Support orientation of new staff and participate in ongoing education and staff meetings.
  
• Maintain patient confidentiality and deliver care that respects cultural, emotional, and physical needs.
  
• Perform additional duties as assigned by nursing leadership
