Physical Therapist - Outpatient - (PT)
StartDate: ASAP Available Shifts: 8 D Pay Rate: $2070.80 - $2269.07

PT for IP/OP needed for 20 weeks! Should have both IP and OP exp, 3 years preferred. OP provided at Haxtun Main Street Clinic (115 S. Colorado), IP provided at Haxtun Hospital (235 W. Fletcher). IP services include acute care patients, as well as IP rehab (extended care). Pelvic floor experience is a plus.

Required Qualifications
Physical Therapist, Outpatient
Experience: 3 years
Licenses: PT(Compact Accepted)
Certifications: BLS-AHA(Copy needed)
AND
Physical Therapist, Acute
Experience: 3 years
AND
Physical Therapist, Rehab
Experience: 3 years
SSN Required
DOB Required
Work History: Should not be more than 30 days of gap in entire work history
References: 1 Reference in the last 12 months
