StartDate: 6/16/2025 Pay Rate: $1701.95 - $1864.90

PT needed asap for 13 weeks Patients are scheduled Monday – Thursday 8:00 – 5:15 and Friday 8:00 – 12:30 Facility Setting? Outpatient SC lic rquired 1 year experience Smart Facility ID (Rehab Only): 1287 Yes, this position will require floating between Cheraw OP PT clinic and Pageland OP PT clinic (CC 478J). Number of beds in your facility? NA for OP setting Preferred experience level? Minimum of 1 year in OP setting Is there anyone in the past that has worked for you that you would consider rehiring? Would consider several of our previous travelers EMR for your facility: Epic Scrub colors / dress code: professional/business casual (no jeans, no leggings, no sweat pants, no tank tops or crop tops, must wear closed toe shoes) OR scrubs of teal, Caribbean, wine, white, black, grey, or pink (solid color matching top and bottom) Parking Location: on site at OP clinic

Physical Therapist, Acute
Experience: 1 year
Licenses: PT(Compact Accepted)(Copy needed)
Certifications: BLS-AHA(Copy needed)
OR
Physical Therapist, Outpatient
SSN Required
DOB Required
Work History: Should not be more than 90 days of gap in last 1 year

Preferred Qualifications
Preferred experience level? Minimum of 1 year in OP setting
