Physical Therapist - Home Health - (PT - HH)
StartDate: 6/30/2025 Available Shifts: 8 D Pay Rate: $1984.00 - $2063.00

1. How long is the contract assignment? 13 weeks 2. Schedule of assignment hours? M – F, 8am – 5pm? Plus weekend call every 3 weeks 3. Will this candidate be visiting patients in care facilities or only in patient’s homes? Both 4. Will you want to hire the contractor at the end of the contract or is this truly a contract assignment? If they are a good fit for the team that option may be there. 6. What region are you in? Home health or Hospice? Home Health 7. What’s creating the request? - work load; one full time PT going to PRN status. 8. To which location will this person report for assignment, full address, Including the OU (Location Code): Toccoa CC-53 Rock Dr Toccoa, Ga 30557 Please confirm above location is correct and list any other territories that are to be covered by the candidates: This position will cover Stephens County. 9. How many miles per week will the consultant travel? 30/40 a day 10. What are the weekly units of productivity expectations? 30 11. What EMR systems will the consultant be using: HomeCare HomeBase, Oasis, Outlook 12. Will there be any on call? If so, how much? Every 3rd weekend; 13. Do you want the candidate to have home health experience? Yes Home Health experience required.
