PHYSICAL THERAPIST Total number of positions needed: 1 All of the info below must be completed before I can open the req. 1. Who will be the DOO approving the contractors time each week? Shari Gladey 2. What is the iCIMS req # corresponding to this requisition? #2025-40238 3. How long is the contract assignment? 13 weeks is the norm: yes 13 weeks 4. Schedule of assignment hours? M – F 8am – 5pm? Yes and PRN for weekend Ortho SOC in assigned region 5. Will there be any weekend rotation? If so, how often? Per above PRN Ortho SOC in assigned region 6. Will there be any on call? If so, how much? NO 7. What region are you in? Home health or Hospice? Home Health 8. What territory this contractor will be asked to cover? Berkshire county. Specific assigned region is north of Pittsfield but might need to travel to other areas to assist co workers at times. The care center is thoughtful on assignments being appropriate to travel 9. What’s creating the request? - work load Ect.. Steady high volume of PT referrals and PT moving out of the area at the end of July 11.
