Physical Therapist - Home Health - (PT - HH)

Position Title: PTA Total number of positions needed: 1 1) How long is the contract assignment? 13 weeks 2) Schedule of assignment hours? M – F, 8am – 5pm? M-F 8 to 5 3) Will this candidate be visiting patients in care facilities or only in patient’s homes? Both, patient homes and ALF 4) Will you want to hire the contractor at the end of the contract or is this truly a contract assignment? Yes 6) What region are you in? Home health or Hospice? Southeast Home Heath 7) What’s creating the request? - work load 8) To which location will this person report for assignment, full address, Including the OU (Location Code) Please confirm above location is correct and list any other territories that are to be covered by the candidates: Gray Care Center 4274 Gray Highway, Suite C Gray, GA 31032 Covering Monroe County 9) How many miles per week will the consultant travel? 30/40 a day? Weekly? 50 to 60 miles per day 10) What are the weekly units of productivity expectations? 25/30? 32 points per week 11) What EMR systems will the consultant be using: HomeCare HomeBase, Oasis, Outlook 12) Will there be any on call? If so, how much? No 13) Do you want the candidate to have home health experience? Yes
