Start Date: 3/30/26 Bill Rate: $75.00/hr VMS MSP OID: Fees: none Modality: PT Credentialing req: Requirements: License: yes...will wait shift: 40 hours/week, 8-hr day shifts M-F Equipment: adult patients, outpatient environment...private company not associated with a hospital. Sports injuries, pre-post surgery, hand/wrist, mobility, orthopedic Bill GWW: yes OT Bill: 1.5 x On Call: no Billing Cycle: weekly Notice: 30 days Parking: free on site Dress Code: scrubs any color contact: Simon Hargus Maternity leave starting April 2026. Cannot start early Mostly 1 patient at a time. THey might be doubled once a day for an hour
