Travel Physical Therapist Home Health jobs in Concord, CA and the Tri Valley area offer a 13-week assignment with Monday through Friday 8-hour shifts, averaging 36 to 40 hours per week. You will provide in-home physical therapy services, helping patients regain independence and function after injury, surgery, or health changes.
