Travel PT Home Health jobs in San Jose, CA covering Alameda County offer a 13-week contract with Monday to Friday 8-hour days and 40 hours per week. You will provide physical therapy to patients in their homes, working 25-30 points per week without overtime. Responsibilities include evaluating patient needs, developing individualized treatment plans, implementing therapeutic interventions, educating patients and caregivers, and documenting care using WellSky EMR. California PT license, home health experience, and WellSky EMR experience are required1.
