Physical Therapist - Acute - (PT - Acute)
StartDate: ASAP Available Shifts: 8 D Pay Rate: $2376.00 - $2449.00

No more than a 30-day work gap, Skills Checklist completed within past year and MUST HAVE references within the past year, no exceptions! 1+ year(s) of experience, active NM License and BLS required. Duties include comprehensive musculoskeletal assessment, develop and carry out individualized treatment plans, deliver skilled interventions, monitor patient progress, educate patients and family members, coordinate care with other healthcare providers appropriately, and document services in compliance with legal, ethical, and reimbursement standards. Common diagnoses seen are Total joints, back/shoulder/foot pain, some neuro (TBI, stroke, Parkinson's). Could work with patients juvenile-aged through geriatric.

Certifications: BLS-AHA(Copy Needed)
SSN Required
DOB Required
Work History: Should not be more than 30 days of gap in entire work history
References: 1 Reference in the last 12 months
