Travel PT Home Health jobs in Concord, CA and the Tri Valley area offer a 13-week contract with Monday through Friday 8-hour days and 36-40 hours per week. You must have a current California PT license, and home health experience is preferred. You will provide physical therapy to patients in their homes, develop individualized care plans, and document visits using Homecare Homebase EMR. The coverage area includes Livermore, Pleasanton, Dublin, and Walnut Creek. You will collaborate with other healthcare professionals to support patient recovery and mobility.
