StartDate: ASAP Available Shifts: 8 D Pay Rate: $2322.00 - $2393.00

PT- Rehab needed asap for 13 weeks What is the work schedule for this position? Tues – Sat preferably otherwise a minimum of 1 Sat shift/mth Facility Setting? Inpatient Rehab Unit 1-2 years experience CA lic required BLS Number of beds in your facility? 30 Preferred experience level? Inpatient Rehab EMR for your facility: Epic Scrub colors / dress code: no set uniform colors Parking Location: outside of unit in their parking lot

Physical Therapist, Rehab
Licenses: PT-CA
Certifications: BLS-AHA(Copy needed)
SSN Required
DOB Required
Work History: Should not be more than 30 days of gap in last 1 year
References: 1 Reference in entire work history
