Travel PT Home Health jobs in Concord, CA are available for 13-week assignments, working Monday through Friday 8-hour days with 36-40 hours per week. You must have a current California license, and home health experience is preferred. Experience with Homecare Homebase EMR is required. Your responsibilities include providing in-home physical therapy services, conducting comprehensive assessments, and developing personalized treatment plans. You will implement therapeutic interventions, educate patients and families on mobility and safety, and document progress in the EMR. The coverage area extends from Discovery Bay to Pittsburg 12.
