Physical Therapist - Skilled - (PT - Skilled)
StartDate: ASAP Available Shifts: 8 D Pay Rate: $2075.00 - $2139.00

This 13-week Physical Therapist assignment is based in a skilled nursing facility in La Crescent, Minnesota, a picturesque river town known for its apple orchards, scenic bluffs, and outdoor lifestyle. The community offers quick access to the Mississippi River, hiking and biking trails, and charming small-town amenities, while being just minutes from a larger regional hub across the river with expanded dining, shopping, and cultural options. In this role, you will provide evidence-based PT services to adult and geriatric residents in a SNF setting, including evaluations, individualized treatment planning, restorative interventions, functional mobility training, and discharge recommendations. You will collaborate closely with an interdisciplinary team, contribute to care conferences, and maintain timely, accurate EMR documentation. The caseload is primarily long-term care and short-stay rehab patients, with a focus on maximizing safe function, independence, and quality of life. The schedule consists of 8-hour day shifts, with expectations for full-time hours across the 13-week contract, and occasional flexibility for facility needs.
