Physical Therapist - Home Health - (PT - HH)

Travel PT Home Health jobs in Albany, NY offer a 13-week assignment with Monday through Friday shifts from 8 a.m. to 4 p.m., totaling 37.5 hours per week. You will provide in-home physical therapy to patients across Albany County, completing 5 visits per day and documenting care using Homecare Home Base. Required qualifications include a current NY physical therapy license and prior home health experience. Your responsibilities include evaluating patient needs, developing individualized care plans, delivering therapy in patients’ homes, and collaborating with the home health team.
