Client Job Title: Social Worker Education: MSW License required: No Other Requirements: DCP at short term acute hospital experience Responsibilities: Will be preforming discharge planning

Required Qualifications
Social Worker, Acute 8a-430p M-F. Must be open to some weekends.
2-3 years recent acute care hospital experience required.
Recent travel experience required.
No locals – must live 50+ miles away.
