This home health Physical Therapist opportunity is based in Tucson, Arizona, a city known for its more than 300 days of sunshine each year, scenic Sonoran Desert landscapes, and easy access to outdoor recreation. In this role, you will provide one-on-one, evidence-based Physical Therapy services to patients in their homes throughout the greater Tucson area. You will focus on adult and geriatric patients recovering from surgery, illness, injury, or managing chronic conditions. Typical responsibilities include comprehensive evaluations, development and implementation of individualized care plans, therapeutic exercise programs, gait and balance training, home safety assessments, and patient and caregiver education to promote independence and reduce rehospitalization risk.
A typical day includes traveling between patient homes, completing initial and follow-up visits, coordinating with an interdisciplinary team that may include nurses, occupational therapists, speech therapists, social workers, and physicians, and documenting care using an EMR system. Caseloads are structured to allow for thorough, quality visits and appropriate travel time, with an emphasis on functional outcomes that matter to patients in their everyday environments.
You can expect a combination of scheduled and PRN visits depending on patient needs, with flexibility in organizing your day. Shifts generally follow daytime hours, with some variability based on patient availability and agency needs. Weekend or on-call rotations may be required on a limited basis, depending on the specific team structure.
