Travel Hospice Registered Nurse job duties

While job duties may vary depending on the facility you work with, here are some common Travel Hospice RN responsibilities:

  • Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs to develop individualized care plans.
  • Administer medications and treatments to manage pain and symptoms, ensuring patient comfort and quality of life.
  • Monitor patients' vital signs and overall condition, adjusting care plans as needed based on changes in health status.
  • Provide emotional support and counseling to patients and their families, helping them cope with end-of-life issues and grief.
  • Educate patients and their families about the hospice philosophy, disease process, and care techniques to ensure understanding and involvement in care decisions.
  • Collaborate with interdisciplinary hospice team members, including physicians, social workers, and chaplains, to provide holistic care.
  • Document all patient interactions, assessments, interventions, and outcomes accurately and timely in electronic health records.
  • Coordinate care and services with other healthcare providers and agencies to ensure continuity of care and support for patients and families.
  • Assist with personal care needs, such as bathing, grooming, and feeding, to promote patient dignity and comfort.
  • Perform regular home visits or facility rounds to assess patient needs, deliver care, and provide support to caregivers.
  • Ensure compliance with hospice regulations, policies, and ethical standards in all aspects of patient care.
  • Participate in ongoing professional development and continuing education to stay current with advancements in hospice and palliative care.
  • Facilitate family meetings to discuss care goals, advance directives, and end-of-life wishes.
  • Provide bereavement support and follow-up care to families after a patient's death, helping them navigate the grieving process.
