Coordinates patients through the acute care hospitalization Manages all aspects of discharge planning/transition process Performs utilization management and quality screenings In- Patient Case Management is required
Skills:
2 years of acute care experience strongly preferred; Case Management, Managed Care. In Patient Case Management is required
Education:
BSN required and RN license required
