Job Details

Provide psychosocial assessments and clinical social work interventions.
Support patients and families with emotional, social, and behavioral health needs.
Develop and implement appropriate treatment and discharge plans.
Coordinate care and collaborate with physicians, nurses, case managers, and other interdisciplinary team members.
Assist with discharge planning, referrals, community resources, and transitions of care.
Provide crisis intervention and appropriate counseling/support.
Maintain accurate and timely documentation in accordance with facility policies.
Protect patient confidentiality and comply with applicable healthcare regulations.
Float and provide coverage across various hospital units as assigned.
Perform other duties consistent with the LCSW role and department needs.
Day 5x8-Hour (07:00 - 15:30), Mid 5x8-Hour (15:00 - 23:30)

Requirements

LCSW – Required
BLS – Required
Minimum 4 years of experience – Required
Ability to work effectively across multiple clinical units.
Strong communication, assessment, organization, and interpersonal skills.
