Experience
Minimum 2+ years acute clinical nursing experience preferred
Case Management experience strongly preferred
Education
Graduate of an accredited Nursing School/College
BSN or MSN preferred
Licensure & Certifications
Current California RN License
CCM (Certified Case Manager) or ACM (Accredited Case Manager) preferred
Required Knowledge & Skills
Case Management and care coordination
Acute care, home care, rehab, hospice, long-term care, and community resources
Medicare regulations, medical necessity guidelines, and quality outcomes
Familiarity with MCG, MIDAS+, Cerner
Strong clinical assessment and critical thinking
Computer proficiency and data entry skills
Excellent communication and collaboration abilities
Ability to prioritize and manage workload independently
