The Registered Nurse (RN) – Home Care is responsible for planning, coordinating, and providing skilled nursing services to patients in their homes. The RN manages an assigned patient caseload, performs comprehensive assessments, develops and implements individualized plans of care, and monitors patient progress toward established goals.
The position provides safe, quality, patient-centered care while working independently and collaborating with physicians and other healthcare disciplines. The RN also serves as a resource to patients, caregivers, families, and members of the care team by providing education, counseling, and support.
Current Registered Nurse (RN) license in the state of practice: Required
Current CPR or Basic Life Support (BLS/HS-BLS/RQI-BLS) certification from an approved vendor, as required by organizational policy: Required
Valid driver's license: Required
Must be at least 21 years of age or older: Required
