Physical Therapist - Home Health - (PT - HH)

Available Shifts: 8 D
Pay Rate: $1946.35 - $2132.70
13 weeks 2. Monday through Friday, 08 am to 5 pm 3. visiting patients in the home setting 4. Could considered hiring the person if interested6. Home Health- cover 5 counties (Daviess, Ohio, Mclean, Henderson and Webster) 7. Currently PT contractor expires on 04/12/25, he is interested only in an extension of his contractor for 2 weeks only. Have Full time physical therapy that is pregnant and maternity leave probably will start by the end of May 2025. Work Load is creating this assignment 8. 2200 East Parrish Avenue, Suite 103, Building E, Owensboro, KY 42303- phone: 270-852-4811 Fax 866-864-4799- Care Center 2149/147 9. 50 to 100 miles weekly 10. Productivity is 6 points daily and 30 points weekly 11. Home Care Home Based 12. No On Call 13. Home Health Experience is needed
