Physical Therapist - Skilled - (PT - Skilled)
StartDate: ASAP Available Shifts: 8 D Pay Rate: $1842.00 - $1898.00

Physical Therapist needed in Columbia, IL. Columbia offers a peaceful, community-focused environment ideal for both work and leisure. This is a fast paced Skilled Nursing Facility with a high skilled volume. Large gym, and great perm support staff.

Required Qualifications
Physical Therapist, Skilled
Experience: 1 year
PT-IL
Certifications: BLS
SSN Required
DOB Required
References: 2 References in the last 36 months

Qualifications:
Valid and unrestricted Physical Therapist license.
Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited program.
1 year of experience in skilled nursing facility
Strong interpersonal and communication skills.
Ability to work effectively in a team-oriented environment.
Commitment to providing high-quality patient-centered care.

Resume:
Must have candidates current name, no nicknames, or abbreviations. Work history must include name of facility worked, city and state, title, department, and dates employed from the last 7 years. Agency names are not accepted as the employer. Highest level of education listed, including month/year of graduation and degree achieved if applicable.

Skills Checklist
Completed within one (1) year of submittal for specialty where candidate is submitted

Two (2) References
Two (2) professional references from work history occurring within the past three (3) years. One must be from a supervisor (charge nurses are considered supervisors). Reference must include a breakdown of skills and strengths with a rating score. Letters of recommendation will not be accepted.
