Skills Checklist Completed within one (1) year of submittal for specialty where candidate is submitted Two (2) Approved References Two (2) professional references from work history occurring within the past three (3) years. One must be from a supervisor (charge nurses are considered supervisors). Reference must include the full name (first & last name) of the person providing reference.
