Proficiency with Homecare Homebase EMR is required. Your responsibilities include evaluating patients in their homes, developing individualized treatment plans, and helping clients regain mobility and independence. You will conduct 5-6 visits per day, covering Santa Cruz and Watsonville, and document care using Homecare Homebase. Mileage is reimbursed at IRS rates.
