This Home Health Physical Therapist position is based in the beautiful Gulf Coast city of Pensacola, Florida, serving patients in their homes throughout the local community and nearby neighborhoods. As a Home Health Physical Therapist, you will independently travel to patient residences to perform thorough evaluations, develop individualized plans of care, and provide skilled interventions targeted to each patient’s functional goals. Typical diagnoses may include post-surgical orthopedic conditions, neurological disorders, cardiopulmonary conditions, general deconditioning, and balance or gait impairments. You will regularly assess home safety, recommend adaptive equipment, and provide caregiver education to support long-term success. A typical day may include scheduling and completing a series of home visits within a designated geographic territory, documenting findings and progress in the electronic medical record, and coordinating closely with nursing, occupational therapy, speech therapy, and other team members. You will participate in interdisciplinary case conferences, communicate changes in patient status, and adjust treatment plans as needed. While daily visit counts can vary by census and patient needs, caseloads are designed to be manageable, allowing for quality, one-on-one patient care in the home environment. Shifts are generally daytime hours on weekdays, with some flexibility based on patient scheduling and agency needs. Occasional weekend or holiday coverage may be requested to support continuity of care. On-call responsibilities, if any, will be communicated clearly and structured to ensure support from the broader team. This role offers a strong foundation for professional growth in home health, with the chance to refine advanced assessment skills, build expertise in community-based rehab, and develop leadership in care coordination.
