You will perform comprehensive assessments, develop individualized plans of care, and deliver skilled interventions including strength and balance training, gait training, transfer training, and home safety modifications. Typical caseloads are designed to be manageable, with visit volumes tailored to support high-quality care and thorough documentation. You will travel between patient homes, often driving scenic country routes and working with a diverse patient population, primarily adults and older adults recovering from illness, surgery, injury, or managing chronic conditions. You will coordinate closely with an interdisciplinary team that may include nursing, occupational therapy, speech therapy, social work, and home health aides to ensure cohesive, patient-centered care. A typical day often includes morning and afternoon home visits, completion of EMR documentation, communication with physicians and care coordinators, and regular phone or virtual check-ins with your home health team. You will have the opportunity to exercise a high level of clinical autonomy, while still benefiting from a supportive, collaborative structure and clinical leadership.
