In this role, you will provide one-on-one, evidence-based Physical Therapy services to patients in their homes throughout Mount Sterling and nearby communities. Your caseload will primarily include adult and geriatric patients with a variety of medical, orthopedic, and neurologic conditions. You will have the opportunity to build meaningful relationships with patients and families, helping them achieve functional goals in the environment that matters most to them.
Typical responsibilities include performing comprehensive initial evaluations in the home setting, developing individualized, functional treatment plans focusing on mobility, strength, balance, and safety, and implementing therapeutic exercise, gait training, and transfer training. You will perform home safety assessments, recommend durable medical equipment and adaptive devices, and educate patients and caregivers on home exercise programs and fall prevention strategies. Ongoing reassessment, modification of the plan of care, and timely, accurate documentation within the EMR system are core components of the role.
Your typical day will involve traveling between several patient homes within a defined geographic area, scheduling visits to maintain an efficient and manageable caseload. Visit volume is designed to support quality, patient-centered care while meeting home health productivity standards. You will collaborate closely with a supportive interdisciplinary team that may include skilled nursing, occupational therapy, speech therapy, social workers, and home health aides. Regular communication with the team ensures coordinated care plans, timely response to changes in patient status, and shared decision-making.
Shifts generally follow daytime hours on weekdays, with some flexibility based on patient needs and agency scheduling practices. Weekend or holiday rotation may be requested, depending on the needs of the service line, with compensatory time or adjusted schedules as applicable. You will enjoy a high degree of professional autonomy in organizing your day, while still having access to clinical leadership, mentorship, and administrative support.
