This Home Health Physical Therapist position offers the opportunity to practice in and around Owensboro, Kentucky, a welcoming riverfront community along the Ohio River. In this role, you will provide skilled physical therapy services to patients in their homes across Owensboro and surrounding communities. You will work with a diverse adult and geriatric population, including patients recovering from surgery, stroke, cardiac events, orthopedic injuries, and chronic conditions who require functional support and rehabilitation in the home setting. A typical caseload includes patients requiring gait and balance training, strengthening programs, transfer training, fall prevention strategies, endurance improvement, and home safety modifications. Your primary responsibilities will include performing comprehensive initial and ongoing assessments, developing and implementing individualized plans of care, and coordinating closely with an interdisciplinary team to support optimal patient outcomes. You will conduct home safety evaluations, recommend adaptive equipment, train caregivers and family members, and provide clear education on home exercise programs and functional mobility strategies. Documentation will be completed via an EMR platform, with an emphasis on timely, accurate, and compliant charting. Most days will include scheduled home visits within a defined geographic territory, allowing you to build strong, ongoing relationships with patients and families. You will generally see multiple patients per day, with visit lengths varying based on evaluation or treatment status. While exact visit volume can vary, the position is structured to balance productivity expectations with the time needed for high-quality, patient-centered care. Travel is primarily local, with limited extended driving, and routes are planned to maximize efficiency. The home health environment is highly collaborative, with a supportive team culture that includes nursing, occupational therapy, speech therapy, social work, and home health aides.
