This Home Health Physical Therapist opportunity is based in the Cedartown, Georgia area, a welcoming northwest Georgia community known for its small-town character, historic charm, and easy access to outdoor recreation. In this role, you will provide skilled Physical Therapy services to patients in their homes throughout Cedartown and surrounding communities. The position offers a meaningful opportunity to practice at the top of your license while helping patients regain function, independence, and quality of life in the comfort of their own home. You will work with a supportive, collaborative team environment that includes nursing, occupational therapy, speech therapy, and other disciplines, all focused on delivering high-quality, patient-centered care.
Typical responsibilities include performing comprehensive physical therapy evaluations in the home, developing and implementing individualized treatment plans, and providing therapeutic interventions such as strengthening, balance training, gait training, transfer training, and functional mobility work that are tailored to each patient’s unique environment and goals. You will conduct home safety assessments, recommend adaptive equipment or environmental modifications, and educate patients and caregivers on home exercise programs, fall prevention strategies, and safe mobility techniques.
A typical day may include traveling between several patient homes in the Cedartown service area, completing initial assessments, revisits, and discharge visits. You will document visits using an electronic medical record system, communicate regularly with the broader care team, and participate in care coordination activities to ensure continuity and quality of care. Patient caseloads often focus on adult and geriatric patients with conditions such as post-orthopedic surgery recovery, stroke, cardiac and pulmonary diagnoses, deconditioning, and chronic disease management.
Work schedules are generally structured around daytime hours, with flexibility for scheduling visits that best meet patient needs. Visit volumes and patient ratios are designed to allow for thorough, high-quality care and safe travel time between homes. The home health environment allows for a high level of professional autonomy, as you assess, treat, and adjust plans of care in real time based on each patient’s progress and home setting.
