You will coordinate care closely with nursing and other therapy disciplines to help patients remain safely in their homes and reduce hospital readmissions. Visits are scheduled to allow for meaningful, unhurried patient interactions and thorough documentation. The typical schedule offers daytime hours with some flexibility to accommodate patient needs and personal preferences. Depending on agency needs, you may work full-time with a set weekly schedule, or in a PRN/part-time capacity offering additional flexibility. The role may include participation in an on-call rotation for urgent therapy needs, though most work is planned and scheduled in advance. You will use a mobile device or laptop for EMR documentation, and you will receive support from office-based staff for scheduling, case coordination, and regulatory compliance.
