This Home Health Physical Therapist opportunity is based in Chesapeake, Virginia, a vibrant coastal city that offers a welcoming community, beautiful natural surroundings, and easy access to big-city amenities. In this role, you will provide one-on-one physical therapy services to patients in their homes throughout Chesapeake and nearby communities. Typical patients include adults and older adults recovering from surgery, stroke, falls, cardiac events, or managing chronic conditions. You will focus on functional mobility, strength, balance, gait training, transfer training, and home safety to help patients remain as independent as possible in their own environments.
A typical day includes traveling between patient homes, completing comprehensive assessments, developing individualized plans of care, and progressing patients through evidence-based interventions. You will perform home safety and equipment evaluations, educate patients and caregivers on home exercise programs, and partner closely with nursing and other disciplines to coordinate care. Documentation is completed through an electronic medical record system using a laptop or tablet in the field.
Visit volumes are structured to support quality, patient-centered care, with caseloads designed to allow for thorough assessments, hands-on treatment, and complete documentation. You can expect a mix of start-of-care assessments, routine treatment visits, and reassessment or discharge visits, depending on patient needs. Travel is generally localized within Chesapeake and nearby communities, minimizing long-distance driving while still providing variety in your day.
You will join a supportive, collaborative clinical team focused on safe, effective transitions from hospital or facility to home. The home health environment offers professional autonomy while still ensuring access to mentorship, clinical resources, and regular communication with your peers.
