This Home Health Physical Therapist position is based in Pleasant Hill, California, a welcoming community in the East Bay known for its relaxed suburban feel, tree-lined neighborhoods, and convenient access to both urban amenities and outdoor recreation. Pleasant Hill offers a vibrant mix of local restaurants, parks, shopping centers, and family-friendly activities, with nearby Walnut Creek and Concord providing even more dining, arts, and entertainment options. The role focuses on providing high-quality physical therapy services in patients’ homes throughout Pleasant Hill and surrounding areas. As a Home Health Physical Therapist, you will enjoy a high level of autonomy and the opportunity to work one-on-one with patients in their own environments, allowing you to tailor interventions that directly impact safety, mobility, and independence where it matters most. You will conduct comprehensive evaluations, develop individualized plans of care, provide skilled therapeutic interventions, and reassess progress regularly, all while coordinating closely with an interdisciplinary home health team. Typical responsibilities include performing functional mobility assessments, gait training, transfer training, therapeutic exercise, balance and fall prevention programs, home safety evaluations, pain management strategies, and patient and caregiver education. You will help patients recover from orthopedic surgeries, manage neurologic conditions, improve cardiopulmonary endurance, and address chronic disease-related functional decline. The home setting allows you to integrate therapy into daily routines, adapt the environment to reduce fall risk, and empower patients and families with practical strategies they can use every day. A typical day in this position may include traveling between multiple home visits, conducting assessments, providing hands-on treatments, documenting care in a mobile EMR system, and communicating with nurses, other therapists, physicians, and case managers. Patient caseload is generally focused on adult and geriatric populations, with visit lengths designed to support thorough, individualized care rather than rushed encounters. While exact productivity expectations, visit numbers, and territory boundaries are set by the agency, the goal is to maintain a manageable caseload that supports high-quality, patient-centered care. Shifts and hours are usually daytime, with scheduling that can offer some flexibility once a caseload is established. Weekend or holiday coverage may be required based on agency needs, with appropriate rotation. You will be expected to drive between homes in Pleasant Hill and nearby communities, so comfort with local travel and independent field work is important.
