This home health Physical Therapist position is based in the Elizabeth City, North Carolina area, a welcoming waterfront community known as the “Harbor of Hospitality.” Located along the Pasquotank River and just a short drive from the Outer Banks, Elizabeth City offers a unique blend of coastal charm, historic character, and easy access to outdoor recreation. As a home health Physical Therapist, you will deliver one-on-one, patient-centered care in patients’ homes throughout Elizabeth City and surrounding communities. You will perform comprehensive evaluations, establish individualized plans of care, and provide ongoing interventions aimed at improving mobility, safety, and independence. Typical conditions addressed include post-acute and post-surgical recovery, orthopedic and neurologic conditions, balance and gait impairments, chronic disease-related weakness, and functional limitations associated with aging. Your typical day will include traveling between patient residences, completing initial assessments and follow-up visits, educating patients and caregivers, and coordinating with an interdisciplinary team. You will focus on restoring function, preventing falls, maximizing safety within the home environment, and empowering patients to participate in their own care through tailored home exercise programs. Patient caseloads are planned to support appropriate visit lengths, thorough assessments, and meaningful interventions. The care environment emphasizes collaboration and support. Physical Therapists work closely with nursing, occupational therapy, speech therapy, medical social workers, and home health aides to ensure comprehensive, coordinated care plans. You can expect a professional setting that values clinical excellence, open communication, and mutual respect among team members. Field-based leadership, clinical support resources, and robust orientation help therapists succeed in a home-based role and maintain high-quality standards of care. Scheduling offers a level of flexibility typical of home health, with visits generally occurring during daytime hours on weekdays, and occasional weekend or holiday coverage depending on patient needs and team staffing.
