As a Home Health Physical Therapist in this role, you will provide skilled, one-on-one rehabilitation services in patients’ homes throughout Lake Zurich and nearby communities. You will conduct comprehensive assessments, develop individualized treatment plans, and implement interventions focused on improving strength, mobility, balance, and functional independence. Typical patients include adults and older adults recovering from surgery, illness, or injury, as well as individuals managing chronic conditions such as cardiopulmonary disease, neurologic impairments, and orthopedic problems.
Your day may include evaluating new patients, delivering targeted therapy sessions, instructing patients and caregivers in tailored home exercise programs, and collaborating with an interdisciplinary team to optimize outcomes. You will travel between patient homes, adapting treatment strategies to varied home environments and leveraging creative problem-solving to promote safety and independence. Documentation is completed in an electronic medical record system, with an emphasis on accurate, timely charting that reflects skilled PT interventions and patient progress.
Workload and scheduling are designed to support efficient routing and meaningful time with each patient. Visit frequency and patient ratios are structured around clinical needs and regulatory guidelines rather than fixed unit-based staffing, allowing you to focus on quality, individualized care. The home health environment offers professional autonomy, the ability to build strong therapeutic relationships, and clear opportunities to expand skills across a diverse caseload.
