Travel PT Home Health jobs in Williston, VT are available for a 13-week contract, covering Addison County. You will work Monday through Friday, 8-hour days, with 40 hours expected weekly and 6 visits per day. Home Health experience is preferred and you must have an active Vermont license.
Your responsibilities include performing diagnostic tests and measurements, developing and implementing individualized care plans, and documenting visits using Homecare Homebase EMR. You will educate clients and their families on rehabilitative care and activities to promote health, safety, and independent living. Strong organizational and interpersonal skills are essential
