Job Title: Registered Nurse - Home Health

Job Description:
We are seeking experienced Registered Nurses (RNs) for Home Health care in Carroll County with potential support for Baltimore County. This is a full-time, contract position with a Monday to Friday work schedule (8:00 AM - 4:30 PM).
The RN will provide compassionate care to a geriatric patient population, performing home visits to manage and monitor patients' health needs. The expected visit volume is 6 patients per day. This position may require on-call duties and occasional weekend shifts based on patient needs.
Key Responsibilities:
Provide skilled nursing care to patients in their homes, including assessments, wound care, medication administration, and patient education.
Complete Oasis documentation accurately and in a timely manner.
Communicate effectively with patients, families, and the care team.
Monitor and document patient progress, updating care plans as needed.
Provide education and support to patients and families regarding health management and care.
Participate in on-call rotation as needed.

A flexible work schedule may be required for some candidates, including the possibility of weekend and on-call shifts.
Minimum of 2-3 years of home care experience required.
Must have Oasis documentation experience.
Must possess an active Maryland or Compact RN license.
Basic Life Support (BLS) certification required.
Must have a valid driver's license and reliable vehicle for travel.
