Days: 7:00 AM – 3:15 PM (EOW)
Evenings: 3:00 PM – 11:15 PM (EOW)
Nights: 11:00 PM – 7:15 AM (EOW)
Weekend-Only roles available
Interviews: In-person only

Department: Nursing Station 5N
Setting: Long Term Care / Nursing Home
Charting: Paper (No Epic)
Patient Population: Geriatric / LTC
Requirements
License: Active NY State LPN
Experience: Minimum 1 year (LTC/Nursing Home preferred)
Professionalism: Strong communication & reliability required
Job Responsibilities
Provide direct patient care under RN/Physician supervision
Perform assessments, documentation, and patient monitoring
Assist with individualized care plans and patient evaluations
Collaborate with interdisciplinary care teams
Ensure patient safety, dignity, and comfort
