We are seeking an experienced Home Health Registered Nurse (RN) with strong OASIS assessment experience to provide high‑quality, patient‑centered care in the home setting. The RN will conduct comprehensive assessments, develop individualized plans of care, manage caseloads, ensure accurate and timely documentation, and collaborate closely with interdisciplinary teams. This role requires strong clinical judgment, excellent communication skills, and the ability to work independently while following all State/Federal regulations and agency policies.
Key responsibilities include conducting OASIS assessments, implementing nursing interventions, educating patients/caregivers, managing care transitions, coordinating with physicians and team members, maintaining infection control standards, and ensuring timely and accurate documentation of all care delivered.
Active RN license in the state
OASIS experience required (strong understanding of documentation requirements & updates)
Minimum 1 year of Home Health experience preferred
Strong knowledge of evidence‑based nursing practice
Ability to assess needs, plan and coordinate care, and manage caseload efficiently
Proficiency in home health documentation systems (e.g., McKesson or similar)
Experience with high‑tech skills (IV therapy, wound VAC, complex wound care)
Ability to collaborate with LPNs, HCAs, PT/OT, Social Workers, and other IDT members
Strong communication, critical‑thinking, and time‑management skills
Current BLS certification
Valid driver’s license and reliable transportation
