An experienced Licensed Practical Nurse (LPN) is needed for a 13-week assignment in a Long Term Care (LTC) setting. The LPN provides compassionate, high-quality nursing care to residents in collaboration with the interdisciplinary healthcare team. The ideal candidate is dependable, organized, and skilled in providing care for adult and geriatric populations in a fast-paced environment.
Responsibilities:
Provide direct patient care including medication administration, treatments, and monitoring of resident status.
Assist with daily living activities, wound care, and documentation of patient progress.
Communicate effectively with RNs, CNAs, physicians, and families to ensure optimal care.
Observe and report changes in residents’ conditions to supervising nurse or physician.
Maintain accurate and timely documentation in the electronic health record.
Adhere to infection control, safety, and facility policies.
Support residents’ emotional and physical well-being while promoting dignity and independence.
2 years or more experience in LTC as an LPN/LVN
